Background: Chronic rhinosinusitis with nasal polyps (CRSwNP) is predominantly type 2 inflammatory disease of the upper air ways. A subgroup of patients with CRSwNP suffering from intolerance to nonsteroidal anti-inflammatory drugs (NSAIDs) and developing NSAID-exacerbated respiratory disease (NERD). The purpose of this study was to compare by nasal secretion of inflammatory cytokines endotype CRSwNP patients with and without NSAIDs intolerance.
Method: 20-6 collected nasal secretions of patients suffering from CRSwNP, thirteen with NERD and thirteen without NSAIDs intolerance. As a control, nasal secretions were collected from a healthy donor fifteen. Tryptase and ten Human cyto- kines analyzed: interleukin ( IL ) – 4, IL -5, IL – 6, IL -8, IL – 12p70 , IL -13, IL -17A, IL -23, IFN-g and TNF-a by cytokines multiple arrays on the Luminex 200 platform.
Results: Grade of polyposis and more severe operating frequency of polyps in NERD- compared with non-NERD patients. IL-6 and IL-5 in CRSwNP significantly increased compared to the healthy volunteers. IL-5 and IL-13 were significantly increased in subjects suffering from NERD compared with patients without NERD CRSwNP. Conclusion: We identified IL-13 as a biomarker may be limited in the nasal secretions of patients with NERD, which allows us to distinguish between CRSwNP with vs without NERD. Characterization endotypes inflammation in CRSwNP allow the introduction of the best available therapy in the context of precision medicine.

immune-mediated disease characterized by the abnormal activity of the immune system. B-induced membrane vesicles Cytochalasin (CIMVs) is an innovative therapy instrument. However, the immunomodulatory activity of Human mesenchymal stem cell (MSC) -derived CIMVs (CIMVs-MSCs) are still unknown.
Therefore, we sought to investigate the immunological properties of CIMVs-MSCs and evaluate its effect on Human peripheral blood mononuclear cells (PBMC). We found that CIMVs-MSCs mainly uptaken by monocytes and B-cells. In addition, we show that CIMVs-MSCs inhibit phytohemagglutinin (PHA) induced proliferation of PBMC, with a more pronounced effect on the expansion of T-lymphocytes compared to the B-cell. In addition, the activation of T-helper (CD4 + CD25 +), B-cells (CD19 + CD25 +) and T-cytotoxic lymphocytes (CD8 + CD25 +) were also significantly suppressed by CIMVs-MSC.

Additionally, CIMVs-MSCs decrease in the secretion of a factor of epidermal growth (EGF) and fractalkine pro-inflammatory in the population of PBMCs, while the release of FGF-2, G-CSF, anti-inflammatory GM-CSF, MCP-3, anti-inflammatory MDC, anti -inflamasi IL – 12p70 , a pro-inflammatory IL -1b, and MCP-1 increased. We analyzed the effect of CIMVs-MSCs in isolated populations of CD4 + and CD8 + T-lymphocytes and show a different immune response and cytokine secretion. Finally, we observe that there is no xenogeneic or allogeneic transplantation of CIMVs induced immune responses in mice. Our data show that CIMVs-MSCs have immunosuppressive properties, is a potential agent for immunomodulatory treatment, and worthy of further investigation.
